Assistive Technology Assessment Forms:
The following is an Assistive Technology Assessment package from Wisconsin's AT Initiative. It is a very comprehensive A.T. assessment package covering areas such as
computer access, communication, writing, math, handwriting, vision, hearing, mobility and a few more. All members of the student's IEP should contribute information for the best assessment. The AT consultants at IISD use this package as well as the SETT framework to determine the best technology to have students try.
You can use the typewriter tool in Adobe Reader to complete any form in the package. Also included is an A.T. checklist, a great starting place.
